CO-18: Duplicate Claim — Meaning & What to Do

CO-18 tells you the claim is an exact duplicate of one the payer already processed. It's a common result of a provider submitting the same claim twice — or submitting it again because the first one seemed stuck. Before anyone resubmits, confirm whether the original claim already paid.

What to do next: If the ORIGINAL claim was denied and the resubmission is what's showing CO-18, the correct move is a corrected claim (with a different claim ID), not an appeal. If the original never paid and the payer still reports a duplicate, call the payer's claim line with both claim numbers.

Did my claim get paid or denied?

CO-18 only tells you the duplicate line is being ignored. Check the ORIGINAL claim number on your EOB — that's the one that matters. If it shows paid, you're done.

Should I resubmit if I see CO-18?

No — first find out what happened to the original claim. Blindly resubmitting creates a third claim and more delays. Ask the payer for a claim status inquiry.

Can two different offices both bill me for the same service and claim it's not a duplicate?

Yes — a facility fee and a physician fee are separate legitimate charges. If you see two providers for one visit, ask both for their CPT codes and verify you're not being billed twice for the same line.
